Utah places Maple Lake Academy’s license on conditional status

SPANISH FORK, Utah — A Spanish Fork treatment center, Maple Lake Academy, runs the risk of losing its license over the death of a teenager last month.

The Utah Department of Human Services, Office of Licensing, issued a Notice of Agency Action against Maple Lake Academy, placing its license on conditional status effective Monday.

This comes following the death of a teenager on Jan. 16 at the small residential program in Utah County. State officials say her death was due to a medical issue.

According to the Notice of Agency Action, the victim reported symptoms from an illness getting worse for at least a week before her death. And after repeated requested from the parents, the program took the individual to the doctor for an examination. After the doctor visit, the individual continued to complain of symptoms that were getting worse. The Note of Agency Action says the individual passed away the next day. 

Utah County Sheriff’s Sgt. Spencer Cannon told KSL that he can’t give a lot of details. However, they responded to the academy group home on Jan. 16. Emergency personnel transported the teenage girl to the hospital, but she did pass away. They are still investigating causes, Cannon said.

Providing help

According to its website, Maple Lake Academy is a program designed to help students with possible learning difficulties, spectrum issues or communication and social shortcomings.

The Office of Licensing has the authority to deny, place conditions on, suspend, or revoke a human services license if the office finds the program has failed to comply with the licensing rules.

In the case of the Maple Lake Academy, the Office of Licensing has found 10 issues that must be resolved before it will have its license removed from conditional status. Among those, Maple Lake Academy can no longer accept clients while the Notice of Agency Action is in place. Additionally, the academy must provide its clients with prompt and necessary medical care.
